Hi Sukhipatiala
Even if they took your insurance information, it was not a written conversation and so you do not have any proof. You send a debt validation letter along with your insurance details by certified mail. Wait for 30 days after they have received your mail. I think they will not be able to validate the debt. If they cannot validate your debt, send them a dispute letter by certified mail to the CRAs asking them that you do not agree with the listing. I think the listing will be removed from the credit report

Your credit score has fallen by over 150 points for just 490$. I think that there may be some other listings too along with the NCO MEDCLR listing, which has collectively affected your credit score so much. If you have sent the insurance documents earlier and have the proof, dispute the listing directly with the CRAs. Else you may sent a written request to NCO MEDCLR along with your insurance documents and request them to remove the listing.

I also agree that you need to pull your CS from the 3 CRAS b/c that drop is a bit to large... If I were you I would call your insrance company and see if they did payment and if possible get a reciept. I would then call the OC and explain the issue to them and see if they could help you since it is a paperwork error on their part.

Just for 490$, a fall in credit score of about 151 points is what I too think is a bit large. You contact your insurance company and see whether they have already paid or not. If they have paid, get that in writing and then dispute the listing with the CRAs. If the insurance company has not paid them, try to come to a PFD to get the listing removed as soon as you pay back the debt.
