With a pay for delete, do I have to pay off all the collection agencies, even if they are for the same original creditor? Do I have to pay the original creditor and then ALL the collection agencies under that creditor?

You would pay off the collection agency that has it now, not the previous collection agencies. You would also have to write letters to the credit bureaus letting them know which CA was paid and tell them to have any other collection agencies state on the report "transferred to another agency and paid".
