I have several accounts that are reported for 99 months, 60 months etc.. I have had two late payments on one that is more than 2 years old and one late payment that is 6 years old. At what point do they "fall off" or do not adversely effect your score? Do I have to request that they be removed?

Any kind of negative items will remain on your credit report for 7 years from the date of your first default. You may make a written request to your creditor/collection agency to remove those items. But it depends on your discretion whether or not to accept your request.
