I have a negative item marked on my credit report with an account number but no account name or address. How do i either dispute it or have it removed from report?

To have it removed you will have to go through the dispute process. If you want some examples of dispute letters you can get there here under letters of credit. Look to the side of your computer screen and you will see Financial tools. Click on the Letters link and you will find them there.

You can send a letter by certified mail to the credit bureaus mentioning your account number asking them to provide you with the details of the creditor or the collection agency who claims to owe that debt. The bureaus will provide you with details of the creditor. If the bureaus do not reply to your request, you can send file a complaint with the FTC. Address of the creditor is generally not mentioned on the credit report, but name of the creditor is always mentioned along with the account number.
