So something told me to randomly check my credit report and I realized the Los Angeles Fire Dept had charged me for an ambulance ride that occurred in February of 2012.

I received NO bills in the mail. I didn't see where they had tried to bill my insurance company. I didn't receive any phones etc.

I see that as of October 2012, the bill was sent to a collection agency and I owe approximately $1400. What in the....?

Would that be a better option?

My wife and I are currently enrolled in a debt management program and after reading that a debt management program may be just as bad as bankruptcy I am a little concerned. The debt management company charges me 55.00 per month to handle my debt. What they do I myself can do that on my own by negotiating with the creditors on my own. Would that be a better option or should I just stick with the debt management company?

Is this normal?

Regarding debt consolidation, my wife and I have an eviction from Feb 2012 and the attorney firm collecting for the debt wants us to complete a form with employer info, bank info, etc before they will make payment arrangements with us or tell us what they might accept as a minimum payment. We don't necessarily want to release that info. Is this normal and can debt consolidation help or reduce the amount owed?
